Why does a chain fall off?

On the way home yesterday, took a left turn (i.e. was going slow) and accelerated up a gentle hill when the chain came off!!  :eek7: Why? ???

Took it home and found the following:
No damage to sprockets :thumleft:
No damage to chain (Chain & sprockets have done less than 10 000 km) :thumleft:
Chain tension within specifications :thumleft:
However, close inspection of the tensioners showed that the back wheel was slightly (about 2-3 mm) out of line - i.e. R/H axle bolt was 2-3 mm ahead of L/H axle bolt.  ::) Corrected it, which also increased the chain tension slightly, and all seems fine.

Anthing else I should be aware of/look out for/be scared of?:lamer: Surely, chains should not be coming off randomly!!

What say you dogs?
