Dealing with toll gates on a bike

Does anybody have advice on dealing with toll gates on a bike?  The whole idea of stopping, putting in in neutral, getting out my wallet etc. seems a bit tedious - especially with my injured left shoulder and restless cages behind me.  Is it best to use cash or a card?

I'm sure somebody has found an elegant solution?
