Chain adjustment

When you adjust your chain, we all know that those little numbers need to align where you turn the screw to tighten or slacken the chain. Sorry, I'm not sure what you call that. My question is, do you have to both sides to the milimeter, or can you have a slight difference...say 2-3mm. Also what will the consequences be for not aligning it properly?
