Check out "Global Rescue", a company dedicated to saving one's ass.  Not medical insurance per se, but will cover first class primary medical care and evacuation costs to your home hospital.  Anywhere on the planet, is my understanding.

I've learned the petrol station lesson already.  Not me, but I've become a Trusty Sidekick...one who tows motos with empty gas tanks, apparently.  All the way to permanently closed gas stations!  Nothing that a 200 km round trip with a gas can can't remedy😨 Sent from my Nexus 6P using Tapatalk

Watch for scam artists at the CA and SA birders who will try to sell you "passes" and multiple country insurance schemes.  Don't fall for it.  They are very convincing, have fake badges and fake papers but just FYI.  They are VERY PERSISTENT and you will have to decline forcefully and rudely. ...
