A friend of mine made me this bolt on extender from aluminium. It bolts on to the brake pedal with NO holes needed to be drilled in the pedal. It raises the pedal by approximately 20mm and also makes it a bit wider.
He is willing to make some more using aluminium bar at a cost of R 120 including postage and the 3 nuts and bolts used to secure it.
