My XR400 makes this tapping/knocking from around the top end.
I do not think it has always done it but can't remember when it started, and sure its getting louder.
Once the bike is completely warmed up it goes away and some days the noise is louder than others.

What year is your XR? The 1996-2000 models clearance is 0.081-0.0121mm for intake valve and 0.106-0.147mm for the exhaust valve.
I also thought clearance was measured under the nut - it's not - 'between the rocker arm pad and the valve stem' is where.
But here's my experience...
The bike was running fine before, it is just maintainence.
The manual does say that you must never turn it clockwise because the decompression system works one way only :thumleft:

I have been using the 'little' ratchet - didn't want to play with my (big) brand new torque wrench unless really necessarry.
I still managed to make marks/grooves on the nut from the socket :P (not from it slipping off)
